Registration information

To be eligible for HealthSelect MA PPO plan, retirees, survivors and their dependents must:

• be at least 65 years old OR certified as disabled by the Social Security Administration (SSA),
• not actively be working
• be entitled to Medicare A (hospital care) and enrolled in Medicare Part B (other medical) coverage and
• living in the United States, the District of Columbia or any of the U.S. territories.
