Dental coverage for your oral health needs
With UnitedHealthcare Dental®, you’ll have access to the largest national Medicare network in the industry, with more than 103,000 providers.
- 100% coverage for exams, x-rays, cleanings and periodontal maintenance, when you see a network provider
- 80% coverage for minor services, including fillings, pulp protection and nitrous oxide*
- 50% coverage for major services, including crowns, root canals, dentures and more*
- $50 deductible, $1000 annual maximum. Deductible and annual maximum do not apply to preventive and diagnostic services.
- Option of seeing out-of-network providers, if desired. You may pay more when going out-of-network.

Vision Eye Exam and Eyewear
Take advantage of comprehensive eye exams and eyewear benefits worth looking at
With the vision benefit, you'll have access to a nationwide network of providers with the freedom to see any participating vision provider. You will have access to an annual routine eye exam through a vision provider and an allowance toward eyeglasses (frame and lenses) or contacts for vision correction, not related to cataract surgery.
- A routine eye exam once every 12 months with a $0 copay
- $130 allowance toward eyeglasses (frames and lenses), every 24 months
- $175 allowance toward contact lenses instead of eyeglasses, every 24 months
- When scheduling your appointment, make sure your vision and eyewear provider(s) will bill the UnitedHealthcare medical plan before receiving routine vision services (routine eye exam and eyeglasses or contact lenses)
- Out-of-network providers may require you to pay upfront and submit a reimbursement claim to UHC.
The network is UnitedHealthcare Medical Network with information on your UnitedHealthcare member ID card.
Some providers may ask you to present a different ID card for your vision benefits. Your vision benefits are part of your medical plan and the same ID card you utilize at a hospital, doctors office, or pharmacy is what you will present to the vision provider. If they have questions, they may call the number on your ID card.
You have the flexibility to use an out of network provider. If the provider does not accept insurance plans or refuses to bill UnitedHealthcare, you may pay out of pocket and request reimbursement. You will be reimbursed the full amount paid up to the allowance amount.

Network size varies by market. Other hearing exam providers are available in the UnitedHealthcare network. You must contact UnitedHealthcare Hearing prior to using your hearing aid allowance. The plan only covers hearing aids from a UnitedHealthcare Hearing network provider.

HouseCalls may not be available in all areas.
With this benefit, you'll get a credit each quarter added to your UCard to buy covered OTC products from network retail locations. The credit is automatically loaded to a prepaid card every quarter. All credits will expire annually.
Renew Active® is the gold standard in Medicare fitness programs, offered at no additional cost to you.
Renew Active includes:
- A free gym membership at a fitness center you select from our large nationwide network. Visit UHCRenewActive.com to find a fitness location near you.
- Thousands of on-demand workout videos and live streaming fitness classes.
- Social activities at local health and wellness classes and events.
You will need a confirmation code to activate your gym membership. Once you’re a member, you can sign in to your plan website, look for My Coverage and select Access gym code. Or you can call the number on your UnitedHealthcare member ID card to ask for a code.
Participation in the Renew Active® program is voluntary. Consult your doctor prior to beginning an exercise program or making changes to your lifestyle or health care routine. Renew Active includes standard fitness membership and other offerings. Fitness membership equipment, classes, personalized fitness plans, caregiver access and events may vary by location. Certain services, discounts, classes, events, and online fitness offerings are provided by affiliates of UnitedHealthcare Insurance Company or other third parties not affiliated with UnitedHealthcare. You must have a working landline and/or cellular phone coverage to use PERS.
With UnitedHealthcare Healthy at Home you are eligible for the following benefits up to 30 days following all inpatient and skilled nursing facility discharges*:
- 28 home-delivered meals when referred by a UnitedHealthcare Engagement Specialist
- 12 one-way rides to medically related appointments and to the pharmacy when referred by a UnitedHealthcare Engagement Specialist
- 6 hours of non-medical personal care provided through a professional caregiver to perform tasks such as preparing meals, bathing, medication reminders, and more. A referral is not required.
*A new referral is required after every discharge to access your meal and transportation benefit
When you use one of the approved meters and corresponding strips, your cost-share for diabetes testing and monitoring supplies is a $0 copay.
These supplies also include any brand of lancets, lancing device, glucose control solution (to test the accuracy of your meter), and replacement batteries for your meter.
To switch to one of the preferred brands, you may be required to get a new prescription from your doctor. A temporary supply of your current brand can be requested.
