To enroll in UC Medicare Choice plan

If you are enrolled in another Medicare plan, UC’s annual Open Enrollment is your opportunity to review your current plan and make a change to another plan - including UC Medicare Choice.

Turning 65 next year

You should participate in the UC’s annual Open Enrollment and enroll in the UC Blue & Gold plan, which is the partner plan, then transition to UC Medicare Choice, typically the first of the month of your 65th birthday.

  • If you are eligible for Medicare premium-free Part A, UC requires that you enroll in both Part A and B, and transition to the Medicare version or the partner plan of your UC medical plan when you turn 65.  
  • Three months before to your 65th birthday, you should receive information from UC about enrolling into Medicare and the required UC’s Medicare assignment UBEN 121 form.  Form should be received by UC’s Retirement Administration Service Center (RASC) no more than 90 days before or no later than one month to your 65th birthday, to avoid delay in your enrollment into UC Medicare Choice.  Contact UC RASC at 1-800-888-8267, 8:30 a.m. – 4:30 p.m. PT, Monday through Friday if you have any questions.
  • Your family members, who are under 65 and not enrolled in Medicare yet, will remain or transition to the UC Blue & Gold plan, which is the “partner plan” to UC Medicare Choice.

What happens after UC Retirement Administration Service Center (RASC) sends the request for enrollment in the UC Medicare Choice plan?

  1. We will review your information and eligibility.
  2. Your verified application will be sent to the Centers for Medicare & Medicaid Services (CMS) for approval. Approval may take up to one week.
  3. Once your enrollment has been approved by CMS, you will receive your confirmation letter, Plan Detail Booklet and your plan member ID card. 
  4. Click the “Sign in or register” link above for quick and easy access to view plan documents, find providers and pharmacies in your area, look up your Drug List (formulary) and review your Evidence of Coverage (EOC). 
  5. Be sure to update your profile with your current address and phone number.
