Enrollment Information
You will automatically be enrolled in the UnitedHealthcare Group Medicare Advantage (PPO) - MA plan, sponsored by International Motors. You don't need to fill out an enrollment form or take any further action.
Opt-out
If you do NOT want to be enrolled in the plan, call the P&C Connection Network at (877) 353-5100.
Deadlines
You must opt out by phone, no later than November 17, 2025. If you don't opt out by the deadline, you will automatically be enrolled in the UnitedHealthcare Group Medicare Advantage (PPO) - MA plan.

What happens after I enroll?
- Once CMS approves your enrollment, you'll be sent a confirmation letter and your plan member ID card.
- Soon after, you'll receive your Quick Start Guide in the mail. It includes details about your specific health plan benefits.
- Watch for your Annual Notice of Changes (ANOC), which is mailed to you every year you're a plan member. It describes your benefits, and any changes to them, for the next calendar year.
