Enrollment Information


The enrollment period for the UnitedHealthcare Group Medicare Advantage (PPO) plan or the Medicare Plus plan by UnitedHealthcare (Senior Supplement) plan, sponsored by the State of Wisconsin Group Health Insurance Program, was  September 25 – October 20, 2023. Review your Health Benefits Decision Guide and visit etf.wi.gov for more information about enrolling in the It’s Your Choice Medicare Advantage (PPO) plan or the Medicare Plus plan by UnitedHealthcare (Senior Supplement) plan through ETF.

What are the differences between the plans?

Your group offers more than one plan option to choose from. See the Coverage and Benefits page for an overview of each plan

Who can enroll?

You can enroll in a Medicare Advantage plan if:

  • You are eligible for participation in the State of Wisconsin Group Health Insurance Program
  • You (and all of your covered dependents) are currently enrolled in Medicare Part A and Part B or are newly eligible for Medicare
  • You live in the service area of the plan. All of the U.S. and its territories is included in the plan’s service area

If you're interested in the Medicare Plus Senior Supplement plan, you can enroll if:

  • You're currently eligible for and enrolled in Medicare Part A and Part B.

What happens after I enroll?

After you submit your enrollment request form to ETF, your plan sponsor, here's what you can expect:

  1. We'll verify your eligibility
  2. As a Medicare Advantage potential enrollee, after you're verified, your application will be sent to the Centers for Medicare & Medicaid Services (CMS) for approval. Approval may take up to 7 to 10 days unless additional information is needed.
  3. Once CMS approves your enrollment, you'll be sent a confirmation letter and your plan member ID card
  4. Soon after, you'll receive your Member Kit in the mail. It includes details about your specific health plan benefits
  5. Medicare Advantage enrollees, watch for your Annual Notice of Changes, which is mailed to you every year you're a plan member. It describes your benefits, and any changes to them, for the next calendar year
