The UnitedHealthcare® Group Medicare Advantage (HMO)* plan. Each covered individual must choose a primary care physician (PCP) from the HMO’s network of providers. Some network services—like visits to specialists, physical therapy, speech therapy, and occupational therapy—will require a referral from your primary care provider (PCP) in order to be covered. The PCP you choose will impact which specialists and hospitals you’ll be able to see.

The UnitedHealthcare® Group Medicare Advantage (PPO) is a Preferred Provider Organization plan. You have access to our nationwide coverage. You can see any provider (network or out-of-network) at the same cost share, as long as they accept the plan and have not opted out of or been excluded from Medicare.
