UnitedHealth Group is pleased to offer a Medicare Advantage plan option available for the following of active U.S. based employees (full and part-time) who are eligible for benefits under the UHG, Inc. Group Benefits Plan, Optum Partner Services Health and Welfare Benefits Plan, OptumCare Group Benefits Plan, or Optum Select Group Benefits Plan.

Active employees Medicare Eligible family members:

• Siblings 
• Grandparents 
• Aunts 
• Uncles 
• Same family members listed above through marriage.

Note: UHG Active employee must complete an Attestation form for their family member(s) interested in enrolling into the plan.

Employees Separated (or retired) after 10/1/2019 and their Medicare Eligible family members. Note: Both current Board of Directors and Board of Director members that have left since 10/1/2019 can participate.
