Your plan is a Preferred Provider Organization (PPO) plan. You have access to our nationwide coverage. You can see any provider (network or out-of-network) at the same cost share, as long as they accept the plan and have not opted out of or been excluded from Medicare.

UnitedHealthcare protocol prefers Network Providers to use other network providers. There may be circumstances when providers may engage out-of-network providers to perform services related to a Network Provider’s treatment. Members with a UnitedHealthcare Group Medicare Advantage (PPO) non-differential plan receive the same coverage in and out of network when the out-of-network provider accepts Medicare and the plan.

Providers are prohibited from discriminating against any member, with regard to quality of service or accessibility of services, on the basis of race, ethnicity, national origin, religion, sex, age, mental or physical disability or medical condition, sexual orientation, claims experience, medical history, evidence of insurability, disability, genetic information, or source of payment.
