Enrollment Information

Enrollment in the UnitedHealthcare Group Medicare Advantage (PPO) the plan, sponsored by SchoolCare,  was from 10/2/2023 to 11/17/2023. Enrollment for coverage beginning 1/1/2024 or after, call the SchoolCare office at 603-836-5031, Press 1 for Eligibility & Enrollment.

Enrollment deadlines

Enrollment in the UnitedHealthcare Group Medicare Advantage (PPO) plan was from 10/2/2023 and 11/17/2023 or during your Initial Enrollment Period. Not enrolling during these times could result in you not receiving coverage for the plan year or having to a pay a higher monthly premium for your plan

Who can enroll?

You can enroll in a plan if:

  • You're currently enrolled in Medicare Part A and Part B or are newly eligible for Medicare.
  • You live in the service area of the plan.
  • You meet SchoolCare and your former employer’s retiree eligibility criteria.

What am I enrolled in?

You're enrolled in the UnitedHealthcare Group Medicare Advantage (PPO) plan. To learn more about your plan details, review plan benefits and costs

 

What happens after I enroll?

  1. SchoolCare will review your form and verify your eligibility.
  2. After you're verified, your application will be sent to United Healthcare and the Centers for Medicare & Medicaid Services (CMS) for approval. CMS approval may take up to one week.
    If you've enrolled in a Senior Supplement plan, we'll validate that you're currently enrolled in Medicare Part A and Part B.
  3. Once CMS approves your enrollment, UnitedHealthcare will send you a confirmation letter and your plan member ID card.
  4. Soon after, you'll receive your Quick Start Guide in the mail. It includes details about your specific health plan benefits.
  5. SchoolCare will invoice you for the monthly premiums unless you have set-up an automated payment. Do NOT send any payment to UnitedHealthcare.
  6. Watch for your Annual Notice of Changes (ANOC), which is mailed to you every year you're a plan member. It describes your benefits, and any changes to them, for the next calendar year.
