During the Open Enrollment period you had the choice to change your election to one of the other Eaton MAPD PPO plans. Plan options are based on your eligibility.

If you did not want to be enrolled into the MAPD PPO plan or you want to change your enrollment to another option, you should have contacted the Eaton Service Center at Fidelity at 1-866-328-6601 annual enrollment period was from (October 29 - November 12, 2024, Monday – Friday, 7:30 a.m. – 5:30 p.m. ET).
